Q: Is Identity Certificate mandatory for State/Central Government/PSU employees?
Ans. Yes. Identity Certificate (Annexure-"B") issued by the competent authority in prescribed Performa is a must for these employees for getting their passports, without police verification.
Q: I am working in ICICI Bank? Am I required to furnish the Identity Certificate?
Ans. Only those persons working in companies, which are owned by the State/Central Governments, need to submit "Identity Certificates". Employees of Private Banks are not required to submit "Identity Certificates" for getting passports.
Q: What is the difference between Re-issue and Renewal of a passport?
Ans. Passports are Re-issued after the expiry of their 10-year validity period. If you had been issued a Short Validity Passport (i.e. a passport of validity less than 10 years) apply for renewal to extend the normal validity of the existing passport to 10 years. In the case of Re-issue of passport a normal fee of Rs.1000/- will be charged whereas for extending the validity (renewal) no fee is charged
When applying for Reissue of a passport after 10 years, attach:
(a) Old passport in original with self-attested photocopy of its first four and last four pages, including ECR/ECNR page.
(b) Document mentioned at (1) (d), if applicable. [1(d): Government/Public Sector/Statutory body employees should submit "Identity Certificate" in original (ANNEXURE B) along with Standard Affidavit Annexure I. ]
(c) Document mentioned at (1) (e), if the old passport did not have ECNR stamp or it was issued when the applicant was a minor. [1(e): If the applicant is eligible for "ECNR" attach attested copy of supporting document (see COLUMN 15 of the INSTRUCTIONS AND GUIDELINES FOR FILLING UP THE APPLICATION FORM). ]
(d) If there is any change in address, document mentioned at 1 (a). [1(a): one of the following: Applicant's ration card, certificate from Employer of reputed companies on letter head, water /telephone /electricity bill/statement of running bank account/Income Tax Assessment Order /Election Commission ID card, Gas connection Bill, Spouse's passport copy, parent's passport copy in case of minors. (NOTE: If any applicant submits only ration card as proof of address, it should be accompanied by one more proof of address out of the above categories). ]
(e) If the old passport does not contain spouse name, copy of marriage certificate issued by the Registrar of Marriage or affidavit as per specimen in Annexure `D'. [Annex D: http://passport.gov.in/cpv/ANNEXURED_woman.htm]
Requirement: Apply for a Reissue of Passport, with Change of Address, Marital status. Also include Spouse's name.
Challenge: Charges mentioned for Reissue - 1,000/- , Change of Address – 1,000/-. Does that make my total cost 1,000 or 2,000 ?
Ans: If new address is included in the Reissue, then its not considered as a separate request and only 1,000 for Reissue of passport is charged. If address is changed separately, then another 1,000/- is charged.

18-Aug-09: Called Bangalore One. For Address proof, I need to get last 1 year's bank statement with my current address. Received copy of Bank Statement for the period 01-Jan-08 to 27-Aug-09 from DCB on 2nd Sept 09.
30-Sep-09: (9:30am) Documents Taken for Submission at Bangalore One. These documents were arranged based on my telecom with Bangalore One guys and information from website. However, I was told to get more documents as these were not sufficient:
- Completed Passport Application Form (No.1) (Keep a Xerox with self)
- Old passport
- 2 Set of copies (self attested ?) of Old passport – 1st 4 and last 6 pages (including ECNR page)
- Latest Bank Statement with Address as address proof. For pvt bank, the statement should be stamped / signed by the bank. 1 Xerox of the signed/stamped statement.
- Copy of Gas connection Letter and latest Receipt copy attested by Notary (support for pvt. Bank's address proof). 1 copy of each of the attested copies.
- Letter from Company 2 support the 2 points above!
- To include wife's name, Marriage certificate letter's copy attested by Notary. 1 copy of the attested letter
- 3.5 x 3.5 size coloured photographs (3 Nos)
- Write a letter 2 passport office saying that the Name different (Narendra Rana – in gas connection, v/s Narendra Singh Rana – in passport is irrelevant)
More documents / attestations were required. Got the attestation done through notary in BDA complex, and further copies of documents made in BDA.
30-Sep-09: (3:00pm) Further Documents Required. The Passport application form is filled in duplicate (same information filled twice). One set is sent to the office with most of the original documents or attested copy of documents attached, the other set can do with the attested photocopies and few non attested copies. I'm listing here the complete document set that was finally submitted by me for passport renewal.
- Completed Passport application form (Keep a Xerox for yourself)
- Old Passport
- 2 Copies of 1st 4 and last 6 pages of passport (including ECNR page). (To be self attested in front of the Bangalore One executive)
- Latest Bank statement with current address as address proof. For private bank, the statement and a copy of the statement should be signed and stamped by the bank.
- In case you have been at this address for less than a year, then you need to furnish proofs of the other addresses of residence during the last one year.
- To support the private bank's statement of address(!), 2 Copies of Gas connection letter, the latest gas receipt , + 1 attested copy of the gas receipt (attested by Notary)
- As my name on the Gas connection was different from the name on passport (Gas guys missed my middle name), I had to write a letter to passport office saying that the name difference (Narendra Rana – in gas connection letter – v/s Narendra Singh Rana – in passport – is irrelevant, and my name should be considered from my company's HR letter). Get a copy of this letter attested by Notary.
- To further support the address (!!) and my complete name, a Letter from my company for address proof (1 HR letter is ok, if possible get 2 numbers, as they might want to use one for each set of passport application form)
- To include wife's name, take marriage certificate and a attested copy
- 3.5 x 3.5 size colored photographs (3 Nos). The photo must have only white background.
This total exercise of filling the form (which I had done a day earlier – in front of the Bangalore One executive, and got it verified from him) and submitting the documents takes about an hour (not including the time I spent in getting the notary's attestation, new photos with white background, extra photocopies and arranging for bank's stamp on photocopy on its statement!!).
For attestation of documents, the Notary sits at BDA Complex in Koramangala (Shop No. 22, Ground Floor, BDA Complex, Koramangala
20-Oct-09: Police Verification. Police person called with a file No. Note the file no. as this code is given by police and is used to find your file. Make sure to not carry much money. Paid 50/- as "verification charges" (read bribe). The process can take anywhere from 30 mins to 1 hour depending on number of other cases of passport verification and availability of station in-charge / senior inspector / sub inspector for final clearance of verification done by policemen.
30-Oct-09: Online Check. Status changed to Green. => File cleared for issue of a passport and is in line for allotment of a passport number. Anticipated date of dispatch of passport by post à 10-Nov-2009.
14-Nov-09: Passport Received.
